But than you can never identify why risk factors occur. If you can cross reference check lists you can correlate the duration of the efficacy of a safety measure. For example if a fire extinguisher has a shelf life of a year but every 6 months it fails its safety test you know there's a problem.

Not at all - saves all that unnecessary paper ! Not eveyone has billions of pounds to spend on computer systems you know

When I first started my company we moved into the offices I'd leased and our computers were nicked in the first week.
We've got a wipe board now.

Now if that was the NHS, they would have highly paid consultants to advise you on what new system you needed. A wipe board would not do !
